Finding a Community Healthcare Provider:
The partnership between you and your healthcare provider(s) at University of Guelph Student Health Services will come to an end when you are no longer enrolled in undergraduate or graduate studies.
Changing healthcare providers can feel a bit overwhelming, but it is a necessary step to ensure you try to maintain a continuity in your care and prevent any lengthy disruption to the treatment of ongoing health issues or chronic conditions. For this reason, you are encouraged to first try to return to your former family doctor/family health team if possible.
It's important to understand that managing your healthcare is ultimately your responsibility. Taking an active role in seeking out a new healthcare provider will empower you to make decisions about your health and will make a huge difference in the quality of care you receive.
Below are resources to help you with the journey of finding a care provider who aligns with your health goals. You will need to provide information such as your name, health card number and some basic health information to get started.
